You DO NOT need to be a current Balduzzi Pediatrics patient to get your child vaccinated.

Currently doing the pediatric vaccine, 5-11 years of age. MUST be in that age range, no exemptions.

You will need to pre-register your child(ren) by scheduling an appointment but also completing the online intake form. The vaccine is FREE. Insurance information is used to collect an administration fee from your insurance but you will NEVER receive any bill.
